Before you start planning your next trip, make sure you have your passport ready and that it is not expiring soon.

If your passport is lost, damaged, or about to expire, you must take action ahead of time.

Passport Renewal

  • You may use form DS-82 if ALL of the following apply to you:
    – Your current passport was issued less than 15 years ago– You have the passport in your possession– You were at least 16 years of age or older when the passport was issued– It was issued under your current name (or you can document your name change)

New Passport

  • You must use form DS-11 if ANY of the following apply to you:
    – This is your first US Passport– Your previous passport was issued when you were under 16 years old– Your previous passport was isssed more than 15 years ago– Your previous passport was damaged

    – Your previous passport has been lost or stolen and you have already reported it

Lost / Stolen Passport

  • Apply for a Passport and also report a lost or stolen Passport
    using forms DS-64 / DS-11

    – Your previous passport has been lost and stolen, and you have not reported it– The system will prepare both DS-11 and DS-64 so you can report it and apply for a new passport at the same time.

How to Renew a Passport

Many adults with a previously issued U.S. passport can renew their passport without going to a passport acceptance facility. 

If renewing by mail, you will generally use Form DS-82, U.S. Passport Renewal Application for Eligible Individuals.

Step 1: Complete the Renewal Application

If renewing by mail, complete Form DS-82 and carefully review all information before submitting it.

Step 2: Submit Your Most Recent Passport

When renewing by mail, include your most recently issued passport with your application. Your previous passport will generally be returned to you separately from your new passport.

Step 3: Include a New Passport Photo

Submit one recent color passport photograph that meets current Department of State passport photo requirements.

Step 4: Include Name Change Documentation, if Applicable

If your current legal name is different from the name shown on your previous passport, you may need to provide a certified marriage certificate, divorce decree, or court order documenting the legal name change.

Step 5: Pay the Required Passport Fee

Include the appropriate passport renewal fee and any optional service fees that apply to your application.

Follow the mailing instructions provided with Form DS-82. The mailing address may vary depending on where you live and whether you request routine or expedited service.

How to Obtain a New U.S. Passport

 

If you are applying for your first U.S. passport, or if you do not qualify to renew your previous passport, you will generally need to apply in person using Form DS-11, Application for a U.S. Passport.

Step 1: Complete Form DS-11

Fill out Form DS-11 with your personal, contact, citizenship, and identification information. Do not sign the application until instructed to do so by an authorized passport acceptance agent.

Step 2: Gather Proof of U.S. Citizenship

You must provide acceptable evidence of U.S. citizenship. Depending on your circumstances, this may include an original or certified U.S. birth certificate, a Certificate of Naturalization, a Certificate of Citizenship, or a previously issued U.S. passport.

You will also need to provide a photocopy of your citizenship evidence when required.

Step 3: Provide Identification

Bring an acceptable government-issued photo ID, such as a valid driver’s license or state identification card, along with a photocopy of the identification document.

Step 4: Obtain a Passport Photo

You must submit one recent color passport photograph that meets U.S. Department of State requirements. The photo must be 2 x 2 inches and generally must have been taken within the last six months.

Step 5: Apply in Person

Take your completed Form DS-11, supporting documents, identification, passport photo, and required fees to an authorized passport acceptance facility. Acceptance facilities include many post offices, clerks of court, public libraries, and other designated government offices.

Some facilities require appointments, so it is a good idea to check with the location before visiting.

Step 6: Pay the Required Fees

Passport fees vary depending on whether you are requesting a passport book, passport card, or both, as well as whether you request optional services such as expedited processing.

After your application is accepted, the Department of State will process the application and return your citizenship documents separately from your new passport.

 

How to Replace a Lost or Stolen U.S. Passport

If your valid U.S. passport has been lost or stolen, you should report it to the U.S. Department of State as soon as possible. Reporting the passport protects you from potential misuse and causes the missing passport to be canceled.

Reporting a passport lost or stolen does not automatically replace it. If you need another passport, you must apply for a replacement.

Step 1: Report the Lost or Stolen Passport

A valid passport can be reported lost or stolen online, by mail using Form DS-64, or as part of your application for a replacement passport.

Once a passport has been reported lost or stolen, it is canceled and cannot be used for travel even if you later find it.

You should not report an expired passport as lost or stolen.

Step 2: Complete Form DS-11

To obtain a replacement passport, you must apply in person using Form DS-11, Application for a U.S. Passport.

When completing Form DS-11, provide the requested information about your most recent passport and explain where and when it was lost or stolen.

If you filed a police report regarding the loss or theft, provide a copy with your application. A police report is not generally required simply to replace a lost or stolen passport.

Step 3: Gather Citizenship and Identification Documents

Because you no longer have your valid passport to submit, gather acceptable evidence of U.S. citizenship and identification as required for a DS-11 application.

You may also need photocopies of the supporting documents.

Step 4: Obtain a New Passport Photo

Provide one recent 2 x 2 inch color passport photograph that meets Department of State requirements.

Step 5: Apply in Person

Bring Form DS-11, your supporting documents, identification, passport photo, and required fees to an authorized passport acceptance facility.

Do not sign Form DS-11 until an authorized acceptance agent instructs you to do so.

Step 6: Pay the Required Fees

Replacing a lost or stolen passport generally requires payment of the normal fees associated with applying for a new passport.

If your passport was lost or stolen while you are outside the United States, contact the nearest U.S. embassy or consulate for assistance obtaining a replacement or emergency passport.
